In this webinar, we will focus on expansions in care management opportunities for FQHCs in recent years, including new programs that are being implemented effective January 1, 2024. We will cover first, the chronic pain management service, unveiled by Medicare in CY2023, addressing how health centers can use this service to promote alternatives to medication for patients suffering from chronic pain. Next we will address two new care management-oriented services, community health integration and principal illness navigation, which allow health centers to draw on community health workers as part of their clinical workforce serving Medicare beneficiaries. Finally, we will describe the program requirements for new remote monitoring services implemented in CY2024, remote physiological monitoring and remote therapeutic monitoring. For FQHCs, these services will be paid for in the same manner as monthly care management services.
