On August 11, 2026, the Health Resources and Services Administration (HRSA) issued its long-awaited Scope of Project Policy Manual, replacing the separate scope of project policies previously set forth in Policy Information Notices or PINs.  HRSA also released its responses to the comments on the draft Scope Manual, which can help explain HRSA’s “thinking” regarding some of the new policies and criteria.

Overall, having a cohesive, comprehensive Scope Manual, along with the anticipated changes to the online change in scope request system taking place in November 2026, should clarify and streamline the scope of project process.  Among other things, the new Scope Manual should:

  • Align scope criteria with the requirements in the current Health Center Program Compliance Manual and updated grant regulations;
  • Clarify certain general scope of project principles, such as “service area overlap”, as well as Change in Scope processes and forms, with the overarching goal of simplifying decision-making and reducing administrative burden;
  • Explain when services and activities are provided “on behalf of” the health center and thus included within the health center’s scope of project;
  • Establish specific criteria for new sites that are co-located with other organizations and/or operated by contractors or subrecipients; and
  • Integrate newer policies (such as telehealth and temporary service site policies) into the general scope guidance.

The new Scope Manual is effective immediately for all new change in scope requests and other changes to and reviews of a health center’s scope of project.  Further, HRSA has indicated that it intends to work with health centers to update existing scopes of project starting in 2027.  Nevertheless, whether you are planning on submitting a change in scope request, preparing for an upcoming Operational Site Visit, or conducting ongoing compliance efforts, it is never too early for health centers to familiarize themselves with the new Scope Manual.
