Head Start Forms: Reporting Responsibilities

August 19, 2026

This webinar is included in the Head Start Training Package.

Navigating the various forms the Administration of Children and Families, Office of Head Start requires in the course of applying for Head Start funding, managing the award and monitoring, reporting, and closing out the grant can be a daunting task. Particularly, the form instructions themselves may not always be the model of clarity, and the attestations or certifications warn of dire consequences in connection with untruthful or incorrect information.

This webinar is designed to provide practical guidance on the purposes of common grant forms, from certain portions of the Standard Form (“SF”)-424 and SF-425 to property-related forms SF-428 and SF-429, HHS Form 690 (Assurance of Compliance), and indirect rate documentation, among others. Join Feldesman attorney Phillip Escoriaza to walk through each form and understand the underlying legal requirements.

The on-demand version of this webinar will be available for purchase after the conclusion of the live webinar.
